Adoration Health is seeking a dedicated Home Health Physical Therapist for CO/TX travel coverage. The role involves evaluating and treating patients at home, coordinating with physicians, and promoting patient independence in a supportive setting.

You will work full-time, Monday–Friday, with travel across CO and TX, including Denver and San Antonio, offering competitive benefits and a $5K sign-on. Join a growing team focused on compassionate, patient-centered care.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; hospice/community experience preferred.
• Current license to practice PT in the state of practice.
• CPR certification current.

Responsabilités

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication among the treatment team, patient, and family.
• Document services thoroughly to ensure proper billing.
• Supervise PTAs and PT Aides as appropriate.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license from the state of practice
• Possess and maintains current CPR certification
