Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health Physical Therapist for Colorado Springs to provide patient-centered care in home health settings. The role offers PRN scheduling with up to 20 visits per week and opportunities for growth within a supportive team.

You will evaluate and treat patients, set goals, coordinate with physicians and families, and document services for proper billing. Valid PT licensure and CPR certification are required.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets.
• Minimum of 2 years of PT experience; hospice/community experience preferred.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based on physician orders, including recommendation and procurement of adaptive equipment.
• Establish measurable goals and develop treatment plans to move patients toward independence and function.
• Maintain communication among the treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services provided to ensure proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities as per state regulations.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
