Job summary

BrightSpring Health Services is seeking a dedicated Physical Therapist for a Home Health position based in Colorado. This role entails providing expert, patient-centered care while floating between branches in Colorado and San Antonio, TX. The ideal candidate will have a Bachelor's Degree in Physical Therapy and at least two years of relevant experience.

Qualifications

Office Location: Denver, CO

Coverage area: Float between branches in CO and San Antonio, TX

Schedule: Full Time Monday-Friday 8-5

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelor's Degree in Physical Therapy from a college or university with an accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least 2 years of appropriate experience as a physical therapist; community/hospice experience preferred
• Valid license from the state of practice
• Possess and maintain current CPR certification

USD $60.00 / Hour
