Adoration Health in Grand Junction, CO is seeking a dedicated Home Health Physical Therapist to deliver 1:1 patient care in a home setting. You will evaluate, treat, and develop goals with physician orders, supporting independence and safe function.

This full-time role offers a $2,500 sign-on bonus, flexible scheduling, and robust benefits including medical, dental, and vision coverage, a 401(k) with match, mileage reimbursement, generous PTO, and wellness programs.

Qualificações

• Bachelor's degree in Physical Therapy from an accredited program.
• New grads may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; hospice/community experience preferred.
• Valid state license and current CPR certification.

Responsabilidades

• Set measurable goals and create treatment plans to maximize independence and function.
• Maintain communication among the treatment team, physician, patient, and family.
• Document services thoroughly and timely for proper billing.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
