Job summary

Adoration Health, through its home health division, is seeking a dedicated Home Health Physical Therapist to serve Ashland, AL and surrounding Clay, Cleburne, Randolph, and Tallapoosa counties. The role emphasizes 1:1 patient care, independence goals, and collaboration with a team to deliver high-quality, compassionate in-home PT services.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• 2+ years of PT experience preferred; community/hospice experience a plus.
• Valid license to practice physical therapy in the state where you work.
• Current CPR certification required.
• New graduates may be considered in select markets based on program availability.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including selection of adaptive equipment.
• Set measurable goals and develop treatment plans to maximize independence and function.
• Maintain communication with treatment team, physician, patient and family.
• Document services thoroughly for accurate billing.
• Supervise PT Assistants and PT Aides per state practice acts.

Office Location: Ashland, AL Coverage area: Clay, Cleburne, Randolph, and Tallapoosa counties

Schedule: Full-Time (M-F 8am to 5pm)

Responsibilities

As a Home Health Physical Therapist , You will:

• P erform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
