Job summary

Adoration Home Health in Ashland, AL is seeking a dedicated Physical Therapist to join our home health team. You will provide one-on-one, patient-centered care in a supportive environment and work within a coverage area that includes Clay, Cleburne, Randolph, and Tallapoosa counties.

The role requires a BPT from an accredited program, state licensure, CPR certification, and 2+ years of experience; new grads may be considered in select markets.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New PT graduates may be considered in select markets based on program availability.
• At least 2 years of relevant experience; community/hospice experience preferred.
• Valid license to practice physical therapy in the state.
• Current CPR certification.

Responsibilities

• Perform PT evaluation and treatment based on physician orders and procurement of adaptive equipment.
• Set measurable goals and develop treatment plans to maximize independence.
• Communicate with treatment team, physician, patient, and family members.
• Maintain thorough, timely documentation for proper billing.
• Supervise PTAs and PT aides in direct patient care per state practice act.

Full-Time (M-F 8am to 5pm)

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license from the state of practice
• Possess and maintains current CPR certification
