Job summary

Adoration Health in Kingsport, TN is seeking a Home Health Physical Therapist for a full-time, 8:00 AM–5:00 PM schedule. You will provide 1:1 patient care in the home setting, perform evaluations, develop treatment plans, and coordinate with physicians, patients, and families to optimize function.

Responsibilities include documentation, supervising support staff, and ensuring quality care while maintaining independent work with team support. A valid TN PT license and CPR are required.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New grads may be considered in select markets based on program availability.
• At least 2 years of appropriate experience; hospice/community experience preferred.
• Valid license in the state of practice.
• CPR certification maintained.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including equipment recommendations.
• Establish measurable goals and develop plans to improve independence and function.
• Maintain communication with the treatment team, physician, patient, and family.
• Maintain thorough, timely documentation for proper billing.
• Supervise PTAs and PT aides in patient care per state regulations.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
