Job summary

Adoration Home Health, part of Adoration Health, seeks a qualified Physical Therapist to provide 1:1 home health care in Kingsport, TN. The role emphasizes patient-centered evaluation, hands-on therapy, and coordinated care with families and physicians.

The position offers full-time hours, state-licensed practice, and ongoing opportunities within a growing home health organization. Expect teamwork, independent work, and a supportive environment focused on patient outcomes.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program is required.
• New graduates may be considered in select markets based on program availability.
• At least 2 years of PT experience preferred; community/hospice experience is a plus.
• Valid license in the state of practice.
• CPR certification must be current.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Set measurable goals and develop treatment plans to maximize independence and function.
• Maintain communication among the treatment team, patient, and family as applicable.
• Maintain thorough, timely documentation for proper billing.
• Supervise PTAs and PT Aides in direct patient care as per state practice act.

Office Location: Kingsport, TN

Coverage area: Sullivan, Grainger, Claiborne, Hancock, Carter, Unicoi, Johnson Counties

Schedule: Full-Time, Mon-Fri, 8AM-5PM

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
