Job summary

Adoration Health is seeking a Home Health Physical Therapist to provide patient-centered care in the Austin area. Join a supportive team and deliver 1:1 evaluations and treatments in patients' homes.

This full-time role offers a Monday–Friday schedule, competitive benefits, and opportunities for professional growth through a growing company.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• New grads may be considered in select markets based on program availability.
• At least 2 years of physical therapy experience; hospice/community care preferred.
• Licensed to practice in the state; CPR certification current.

Responsibilities

• Perform PT evaluation and treatment based on physician orders and adaptive equipment needs.
• Set measurable goals and develop plans to restore independence and function.
• Maintain communication with treatment team, physician, patient and family as needed.
• Document services thoroughly for accurate billing and records.
• Supervise PTAs and PT aides in patient-care activities per state act.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
