Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health Physical Therapist to join our Centre, AL team. You will provide expert, patient-centered home health care to patients in Calhoun County.

Join a supportive environment with 1:1 care, flexible scheduling, and opportunities for advancement. Alabama PT license and CPR certification required; new graduates may be considered in select markets.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New PT graduates may be considered in select markets.
• At least 2 years of PT experience; hospice/community experience preferred.
• Valid Alabama PT license and CPR certification.

Responsibilities

• Perform PT evaluation and treatment per physician orders.
• Establish goals and treatment plans to maximize independence.
• Coordinate with treatment team, physician, patient and family.
• Document services timely for proper billing.
• Supervise assistants in patient care as per state act.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
