Job summary

BrightSpring Health Services is looking for a dedicated Home Health Physical Therapist for our team in Austin, TX. The role requires providing expert, patient-centered care, performing evaluations, and developing treatment plans.

Qualifications

• Minimum of Bachelor's Degree in Physical Therapy from an accredited program.
• At least 2 years of experience as a physical therapist, preferably in community/hospice settings.
• Valid license from the state of practice and current CPR certification.

Responsibilities

• Perform physical therapy evaluations and treatment based on physician orders.
• Establish measurable goals and develop treatment plans.
• Maintain communication with treatment team, patients, and family members.
• Document services provided for proper billing.

Responsibilities

As a Home Health Physical Therapist, you will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelor's Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least 2 years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license from the state of practice
• Possess and maintain current CPR certification
