Position Overview

All County Home Health and Hospice is seeking a compassionate and skilled Physical Therapist (PT) for a 13-week contract assignment in San Antonio, Texas. The ideal candidate will provide high-quality, patient-centered physical therapy services to individuals receiving care in their homes. The Physical Therapist will evaluate patients' physical functioning, develop individualized treatment plans, implement therapeutic interventions, and monitor progress toward rehabilitation goals. This role also includes overseeing Physical Therapist Assistants (PTAs) and collaborating with the interdisciplinary home health team to promote patient safety, independence, and improved quality of life.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations and assess patients' mobility, strength, balance, coordination, and functional abilities.
• Develop individualized treatment plans based on patients' needs, diagnoses, and rehabilitation goals.
• Provide physical therapy interventions to improve mobility, strength, endurance, balance, and functional independence.
• Deliver home health physical therapy services in accordance with physician orders and applicable clinical guidelines.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home safety.
• Document evaluations, treatment sessions, progress notes, and discharge summaries accurately in the electronic medical record.
• Oversee and coordinate care provided by Physical Therapist Assistants (PTAs), as applicable.
• Collaborate with physicians, nurses, occupational therapists, and other members of the interdisciplinary care team.
• Ensure compliance with applicable healthcare regulations, documentation standards, and patient care protocols.
• Travel to patients' homes within the assigned service area.

• Work Hours: 8:30 AM 4:30 PM
• Guaranteed Hours: 40 hours per week
• Service Radius: Under 40 miles
• Weekends: No weekend shifts required
• On-Call: No on-call responsibilities
• Orientation: Two days in the office
• Equipment: Work phone provided

• Supervisory Responsibilities: Oversees Physical Therapist Assistants (PTAs)

Qualifications
