Job summary

LivinRite Home Health in Virginia is seeking a Physical Therapist to provide in-home therapy as part of a collaborative care team. You will assess adults and geriatric patients, develop treatment plans, and support families in the home setting.

Candidates should hold a DPT or equivalent, with a current Virginia license, CPR/BLS, and valid driver’s license. A minimum of two years in home health or rehab is preferred, with strong communication and critical-thinking skills.

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent (MPT, MScPT).
• Current and unrestricted Virginia Physical Therapy license.
• Two or more years of experience in home health, acute care, or rehab preferred.
• Excellent communication and critical thinking skills.
• Valid driver's license, active auto insurance, and CPR/BLS certification.

Responsibilities

• Deliver skilled in-home therapy as part of a collaborative care team.
• Evaluate and develop individualized treatment plans for adult and geriatric patients.
• Serve as a key liaison between patients, families, and interdisciplinary team members.
• Document care clearly and thoroughly in our EMR system.
• Help shape the future of in-home rehabilitation.
