Job summary

LivinRite Home Health is seeking a Physical Therapist to deliver in-home therapy to adults and geriatrics in Virginia, part of a patient-centered, collaborative team.

• Virginia PT license (unrestricted).
• Two+ years of experience in home health, acute care, or rehab preferred.

Responsibilities

• Deliver skilled, compassionate in-home therapy as part of a collaborative care team.
• Evaluate and develop individualized treatment plans for adult and geriatric patients.
• Serve as a liaison between patients, families, and interdisciplinary team members.
• Document care clearly and thoroughly in our EMR system.
• Help shape the future of in-home rehabilitation.

What You Bring:

• Doctor of Physical Therapy (DPT) or equivalent (MPT, MScPT)

• Current and unrestricted Virginia Physical Therapy license

• Two or more years of experience in home health, acute care, or rehab preferred

• Excellent communication and critical thinking skills

• Valid driver's license, active auto insurance, and CPR/BLS certification
