Job summary

Granite Peak Home Health in Kalispell, MT seeks a licensed Physical Therapist to join our growing team. You will deliver physician-prescribed PT and coordinate with the care team to enhance patient function in a home setting.

The role requires MT licensure, BLS/CPR, and HIPAA knowledge, with preferred 1 year of home health experience. A flexible schedule and mileage reimbursement are offered.

Qualifications

• Graduate from a physical therapist curriculum approved educational institution.
• Current Montana physical therapy license.
• 1 year home health experience preferred.
• BLS/CPR certification required.
• HIPAA compliance and patient privacy knowledge.

Responsibilities

• Provide physician-prescribed physical therapy.
• Improve or minimize residual disabilities.
• Return patient to optimum functional level.
• Participate in care planning with home care team.
• Supervise personnel as required.
• Complete initial assessment, plan of care, and 30-day reassessments.
• Perform skilled procedures as ordered by physician.
• Consult with physicians on treatment changes.
• Document patient progress and communication with physicians.
• Educate patients and families on home programs.
• Contribute to QA and performance improvement programs.
• Attend care conferences and prepare progress notes.

Qualifications

• Graduate from a physical therapist curriculum approved educational institution
• Currently licensed in the state of Montana
• 1 year home health experience, preferred
• BLS/CPR required for this position
• Knowledgeable with healthcare regulations, HIPAA compliance, and patient privacy requirements
• Knowledge of the local healthcare market and community outreach strategies
• Knowledge of appropriate software including Microsoft Word, Email, Outlook, and Wellsky (EHR)
• Clean driver’s license with a clean driving record

Principal Responsibilities

• Understands and adheres to established policies and procedures
• Provides physician-prescribed physical therapy
• Improves or minimizes residual physical disabilities of the patient
• Returns the individual to optimum and productive level within the patient's capabilities
• Participates with all other home care personnel in patient care planning
• Directs and supervises personnel as required
• Responsible for initial assessment, plan of care, maintenance program development, and modifications and reassessments every 30 days
• Performs all skilled procedures as ordered by the physician
• Consult with physicians regarding changes in treatment
• Writes reports to physicians regarding patient's progress
• Instruct patients and family/significant others in-home programs and activities of daily living
• Participates in in-service programs and presents in-service programs as assigned
• Participates in Quality Assurance and Performance Improvement activities as assigned
• Attends all patient care conferences as scheduled
• Prepares clinical and progress notes
• Assists physician with evaluating level of function
• Helps develop the plan of care and revise it as necessary
• Consult with family and Agency personnel
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition under Agency defined time frames
