Qualifications

Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with the patient’s needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs to facilitate effective return to community activities. The Physical Therapist will also serve as an educator for patients, families, students, and coworkers while performing additional professional duties related to patient care and the development of therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients, utilizing clinical reasoning to develop treatment plans and goals.
• Document medical records clearly, accurately, and timely, ensuring compliance with guidelines.
• Provide care under signed, valid physician orders and within insurance approvals as required.
• Participate in student programs, providing a positive learning environment with proper supervision.
• Create an individualized professional development plan and pursue continuing education and professional growth.
• Adhere to the State Practice Act and maintain professional licensing in good standing.
• Understand and participate in organizational performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an Accredited School.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Valid state license to practice as a Physical Therapist.

Schedule

This position operates on a 5x8.0 schedule.
