Job summary

MedCadre is seeking a qualified Physical Therapist or Physical Therapist Assistant to provide rehabilitation services to patients with diverse physical and functional conditions. The PT will independently evaluate and develop treatment plans, while the PTA will provide treatment under supervision of a licensed Physical Therapist.

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
• Active Physical Therapist license in the state of practice.
• Current BLS certification required.
• Associate degree from an accredited Physical Therapist Assistant program.
• Active PTA license in the state of practice.
• Current BLS certification required.
• Ability to provide treatment under the supervision of a licensed Physical Therapist.

Responsibilities

• Evaluate patients and develop individualized treatment plans within scope of practice.
• Provide therapeutic exercises, gait training, balance activities, manual techniques, and functional mobility training.
• Assist patients with transfers, ambulation, strengthening, and activities designed to improve independence.
• Monitor patient progress and modify treatment interventions as appropriate.
• Educate patients and caregivers on home exercise programs, safety, and injury prevention.
• Recommend and train patients in the use of appropriate assistive devices.
• Maintain accurate and timely treatment documentation in the EMR.
• Communicate patient progress and concerns with the supervising therapist and healthcare team.
• Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals.
• Follow infection prevention, HIPAA, patient safety, and organizational policies.

• Strong clinical assessment, treatment planning, and rehabilitation skills.

Required Qualifications PTA:

• Associate degree from an accredited Physical Therapist Assistant program.
• Active PTA license in the state of practice.
• Current BLS certification required.
• Ability to provide treatment under the supervision of a licensed Physical Therapist.

Preferred Qualifications:

• Minimum 1 year of recent PT/PTA experience preferred.
• Experience in outpatient, acute care, inpatient rehabilitation, SNF, home health, or long-term care settings.
• Experience treating orthopedic, neurological, geriatric, post-surgical, or medically complex patients.
• Strong communication, organization, and time-management skills.
• Experience with EMR systems preferred.
