Job summary

Innovate Rehab and Wellness seeks a Licensed Physical Therapist to join a skilled nursing setting in Holton, KS. This full-time role offers one-on-one patient focus, flexible Monday–Friday schedule, and competitive hourly pay based on experience.

You will evaluate patients, create individualized treatment plans, and work with a collaborative interdisciplinary team to restore movement, function, and quality of life, while pursuing ongoing professional growth and outcomes.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with all regulatory and ethical standards.
