Innovate Rehab and Wellness in Nortonville, KS is seeking a Licensed Physical Therapist (PT) for a full-time, on-site role in a Skilled Nursing Facility. You will evaluate patients, create and lead individualized treatment plans, and help restore movement and quality of life through evidence-based therapies.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Active and valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy including manual techniques and exercises.
• Educate patients and caregivers on home programs and injury prevention.
• Track progress and adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with regulatory and ethical standards.
