All Medical Personnel seeks a licensed Physical Therapist (PT) in Sioux Rapids, IA to plan and conduct treatment programs designed to restore physical function and improve patient outcomes. You will work in an interdisciplinary team and supervise assistants while maintaining thorough documentation.

Requirements include graduation from an accredited school, active state license, BLS certification, and at least 1 year of recent PT experience; benefits include medical/dental/vision on day one,

Qualifications

• Graduate of an accredited school.
• BLS certification required.
• Active state license required.
• 1 year of recent experience as a Physical Therapist.
• Demonstrate professionalism with patients, families, physicians and staff.

Responsibilities

• Plans and conducts segments of treatment programs to restore and improve physical, social, and mental functions.
• Co-leads interdisciplinary patient groups and collaborates with all disciplines.
• Completes evaluations to ensure quality standards and designs individualized treatment plans.
• Provides therapy interventions using approved protocols and communicates care plans.
• Completes comprehensive clinical documentation within established deadlines.
• Supervises assigned assistants and ensures correct supervision.
• Establishes and administers a treatment program with realistic goals.
