Ohio’s Hospice seeks a part-time Hospice PCS to join the Middletown Hospice House IPU in Franklin, OH. The 7:00 am–7:30 pm shift includes every other weekend/holiday rotations, plus UC duties in this role.

You will work with an interdisciplinary team to deliver compassionate patient care, documentation in EMRs, and support for families during critical moments. A driver’s license and clean background are required.

Qualifications

• High School Diploma or GED is required.
• Certified Nurse Aide in Ohio is required.
• CHPNA certification preferred.
• Proficient with electronic medical records.

Responsabilités

• Report to a care team for patient-focused nursing assistance.
• Perform patient care tasks in accordance with physician orders and care plans.
• Maintain accurate documentation in electronic medical records.
• Comply with background check and drug screening requirements.

Connaissances

Proficient computer skills

Background check clearance

Valid Ohio driver’s license

Formation

High School Diploma or GED

Certified Nurse Aide in Ohio

CHPNA certification preferred
