Job summary

A $10,000 sign-on bonus is available for eligible full-time candidates. Responsibilities include evaluating functional status, coordinating with the care team, and documenting progress while adhering to state laws and plan of care.

Qualifications

• Active Physical Therapy licensure in Washington state
• Valid driver's license and vehicle or access to reliable transportation
• Ability to work in home settings with diverse clients

Responsibilities

• Evaluates patient functional status and PT needs within home health
• Provides services within scope of practice and plan of care
• Observes and reports patient response to treatment to supervising nurse/physician
• Treats to relieve pain, restore function, and maintain maximum functioning
• Instructs patient and family on plan of care and progress toward goals
• Visits patients per Plan of Treatment and submits progress notes
• Completes PT plan of care within state guidelines
• Reports outcomes of evaluation and anticipated frequency of care
