Job summary

Aspire Home Healthcare Inc is seeking a skilled Physical Therapist (PT) to join our San Diego team and provide one-on-one home health care. You will evaluate patients, design treatment plans, and supervise care coordination with the broader rehab team to promote independence and safety in the home environment.

We offer flexible scheduling, competitive per-visit rates starting at $85 per visit, mileage reimbursement, and growth opportunities across multiple states.

Qualifications

• Active Physical Therapist license in CA.
• Minimum 1 year of PT experience preferred.
• Current TB test within 12 months and CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate, plan, and carry out individualized home health treatment programs.
• Coordinate care with PTAs and HHAs and clinical team.
• Document patient progress accurately and on time.
