Job summary

Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities in Ohio. You will manage acute changes in condition, participate in advanced care planning, and coordinate with the interdisciplinary team to optimize care.

The role includes prescribing medications per Ohio law, ordering tests, and collaborating with the primary care physician to maintain comprehensive care plans.

Qualifications

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances in accordance with OH Law.
• Order, review and follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular basis to discuss changes in condition and care plans.
• Collaborate with primary care physician.
