Job summary

Wellbound Certified Home Health Agency seeks a dedicated Physical Therapist (PT) Field Evaluator to deliver high-quality therapy in patients' homes across all five NYC boroughs and Long Island. You will assess mobility, self-care, cognition, and independence to support optimal function.

Requirements: Master's degree in Physical Therapy, current NY state PT license, and pediatric experience preferred. Strong communication with patients, families, and the care team is essential.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Prior experience in pediatric settings preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct comprehensive evaluations assessing patients’ functional abilities, medical history, and individualized goals.
• Develop and establish a detailed Plan of Care (POC), including treatment goals, visit frequency, and evidence-based interventions tailored to patient needs.
• Perform assessments related to motor skills, balance, endurance, conditioning, and functional mobility.
• Collaborate with Physical Therapist Assistants (PTAs) by clearly communicating the established Plan of Care and treatment goals.
• Ensure PTAs implement interventions in accordance with the prescribed Plan of Care and within their scope of practice.
• Maintain ongoing communication regarding changes in patient status and make appropriate adjustments to the treatment plan as needed.
• Ensure documentation and clinical practices remain compliant with agency standards and regulatory requirements.

In this role, you will deliver high-quality therapeutic services that support patients’ mobility, self-care, cognition, and overall functional independence. Services are provided in patients’ homes throughout the five boroughs of New York City and Long Island. Your clinical expertise will play a critical role in helping individuals achieve their highest level of function in a familiar and supportive environment.

Location: All 5 boroughs and Long Island

Compensation: $100

Job Type: Fee-for-Service (FFS)
