Job summary

WELLBOUND Certified Home Health Agency seeks a licensed Physical Therapist to provide pediatric and adult services in Assisted Living Facilities across the five boroughs of New York City and Long Island.

Responsibilities include designing individualized plans, promoting mobility and fall prevention, coordinating with the interdisciplinary team, and training caregivers to ensure continuity of care. Will travel between facilities within the service area to deliver patient-centered care.

Qualifications

• Valid New York State Physical Therapist (PT) License (required).
• Strong knowledge of anatomy, physiology, and geriatrics to effectively treat diverse populations.
• Experience in clinics, skilled nursing facilities, assisted living, or home health settings preferred.
• Excellent communication and interpersonal skills to collaborate with patients, families, and healthcare teams.
• Ability to build rapport with patients while demonstrating empathy, patience, and professionalism.
• Strong organizational skills to manage multiple cases while maintaining high-quality care.

WELLBOUND Certified Home Health Agency is actively seeking skilled and compassionate Physical Therapists to provide services to pediatric and adult clients with special needs in Assisted Living Facilities across the five boroughs of New York City and Long Island. This role offers the opportunity to deliver meaningful, patient-centered care while supporting mobility, safety, and overall quality of life in a structured residential setting.

Location: All 5 boroughs and Long Island

Compensation: $100.00 per visit

Job Type: Fee-for-Service (FFS)
