Job summary

WELLBOUND Certified Home Health Agency is seeking an Occupational Therapist (OT) Field Evaluator to join our interdisciplinary team, providing comprehensive home-based evaluations and establishing Plans of Care across New York City and Long Island.

Responsibilities include evaluating functional abilities, creating detailed care plans with visit frequencies, and collaborating with COTAs to implement these plans while maintaining compliant documentation.

Qualifications

• Master's degree in occupational therapy from an accredited program.
• Current state licensure as an Occupational Therapist.
• Experience in pediatric and/or home care settings preferred.
• Strong communication and interpersonal skills with patients, families, and team members.

Responsibilities

• Conduct comprehensive evaluations of patients’ functional abilities, medical history, and goals.
• Develop a detailed Plan of Care with goals, visit frequency, and interventions.
• Perform ADL, motor, and cognitive assessments.
• Collaborate with COTAs by communicating the Plan of Care and therapy goals.
• Ensure COTAs implement Plan of Care within scope of practice.
• Maintain clear communication on patient progress and plan adjustments.
• Ensure timely, accurate clinical documentation.

Job description

WELLBOUND Certified Home Health Agency is seeking a skilled and compassionate Occupational Therapist (OT) Field Evaluator to join our interdisciplinary team. In this role, you will provide comprehensive evaluations and establish Plans of Care that support patients’ cognitive, self-care, motor, and functional abilities. Services are delivered in patients’ homes throughout the five boroughs of New York City and on Long Island, allowing you to make a direct, meaningful impact in a community-based setting.

Location: All 5 boroughs and Long Island

Compensation: $100.00 per visit

Job Type: Fee-for-Service (FFS)

Responsibilities

• Conduct comprehensive evaluations assessing patients’ functional abilities, medical history, and individualized goals.
• Develop and establish a detailed Plan of Care, including treatment goals, visit frequency, and targeted interventions.
• Perform assessments related to activities of daily living (ADLs), motor skills, and cognitive functioning.
• Collaborate closely with Certified Occupational Therapy Assistants (COTAs) by clearly communicating the established Plan of Care and therapy goals.
• Ensure COTAs implement interventions in accordance with the prescribed Plan of Care and within their scope of practice.
• Maintain clear and ongoing communication regarding patient progress, changes in condition, and necessary adjustments to the treatment plan.
• Ensure timely, accurate, and compliant clinical documentation.

Requirements

• Master's degree in occupational therapy from an accredited program.
