Qualifications

• Associate degree in nursing required.
• Bachelor's degree in nursing preferred.
• Current RN license in Ohio; BLS preferred.
• Minimum 2 years of ambulatory/outpatient nursing experience.

Responsibilities

• Provide direct patient care and clinical support in ambulatory settings.
• Develop and implement individualized care plans for complex patients.
• Educate patients and families on conditions, treatments and self-care.
• Collaborate with interdisciplinary team to optimize care and patient experience.
• Document nursing history, assessments and plans of care accurately.
• Coordinate referrals, scheduling, and transitions of care.
• Lead/participate in quality improvement and safety initiatives.
