AbsoluteCARE Medical Center & Pharmacy in Ohio is seeking a Psychiatric Provider to join a team-based care model dedicated to medically complex and socially vulnerable members. The role emphasizes collaboration with primary care, behavioral health, and community health workers to improve outcomes.

The position includes on-site care at one of our Ohio locations and tele-psychiatry support for appropriate cases, with a focus on population health and care coordination within a value-based care

Qualifications

• Licensed and board-certified psychiatrist in the applicable state.
• DEA registration.
• BLS certification.
• Experience in population health management is preferred.
• Experience with Medicaid beneficiaries is preferred.
• Experience in addiction medicine is preferred.
• Proven ability to work in a multidisciplinary team.
• Proficient in EMR systems.

Responsibilities

• Guide behavioral health treatment for a cohort via a collaborative care model.
• Collaborate across care teams for high-risk patients.
• Develop educational materials for the care team.
• Provide direct patient care including diagnosis and treatment plans.
• Deliver tele-psychiatry to appropriate members.
• Provide peer-to-peer consultations as needed.
• Support transitional care management and crisis intervention.
• Coordinate with inpatient/outpatient providers for care continuity.
• Conduct post-discharge visits for psychiatric patients.
• Support medication management including LAIs.
