Qualifications

• Master of Social Work required.
• Current license to practice as a Master Social Worker in New York State or as a Clinical Social Worker required.
• 1–3 years of relevant experience required.
• In patient psychiatric experience highly preferred.

Responsibilities

• Provides individual and group social work services to patients and/or families.
• Utilizes psychosocial assessment skills to guide care and identify barriers to access and outcomes.
• Provides interdisciplinary team with assessment findings and potential impact on care.
• Delivers psychoeducation, counseling, and crisis intervention to patients and families.
• Develops and implements treatment and discharge plans aligned with site expectations and coordinates with post-acute providers.
