Job summary

Northwell Health is seeking a per diem Social Worker to provide individual and group services on the psychiatry unit and related medicine areas. You will conduct psychosocial assessments and support patients and families.

The role requires MSW, NY license, 1–3 years experience, inpatient psych preferred; per diem schedule includes weekend shifts once a month and potential weekday coverage; strong advocacy and care coordination encouraged.

Qualifications

• Master of Social Work required.
• Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in New York State required.

Responsibilities

• Provides culturally competent direct service to patients and their families using a variety of social work modalities to maximize functioning.
• Uses high-risk criteria and referrals to identify patients needing assessment and intervention; advocates to ensure access to health care.
• Provides interdisciplinary team with assessment findings and recommended impact on care.
• Offers psychoeducation, counseling, and crisis intervention to patients and families.
• Develops and implements treatment and discharge plans aligned with site expectations and post-acute communication with providers and payers.
• Incorporates community resources and entitlement programs into planning.
• Maintains patient care documentation as per site policies.
• Represents department in hospital/site committees and work groups.
• May supervise social work students and evaluate their field performance.
• Coordinates with program management on student training and placement.
