Qualifications

• NCI-APS certification must be obtained within 90 days of hire for employees in UC Health Psychiatric Services.

Responsibilities

• Respond to referrals and assess psychosocial and mental health needs of patients and families.
• Develop and implement a plan of care addressing social, emotional, financial and developmental aspects of care.
• Provide education and consultation to the healthcare team and act as liaison to patients and families.
• Facilitate discharge planning and coordinate transitions to the next level of care.
• Maintain timely documentation and participate in quality and safety initiatives.
