Job summary

Responsibilities

• Assess patient condition on admission and during each shift, identifying changes
• Perform procedures, monitoring, or other functions as ordered by medical providers and document care in the EMR
• Respond promptly to patient requests and anticipate needs
• Coordinate with multidisciplinary team to customize and deliver a holistic treatment plan
• Act as interface between healthcare providers and community support systems for ongoing care
