Nexus Pavilion at Belleville is seeking a Psychiatric Rehabilitation Services Coordinator to coordinate mental health and psychosocial support for residents, partnering with clinical staff and community providers to support recovery-focused care.

The role focuses on reviewing treatment plans, promoting active resident participation, and coordinating rehabilitation services to improve emotional well-being and independence.

Qualifications

• Higher Education degree in human service profession.
• Demonstrated sensitivity and ability in working with residents, families, staff and community agencies.
• Ability to present concise, meaningful written reports describing social/psychological needs.

Position Overview:

Coordinate mental health and psychosocial support services to help residents improve emotional well-being, coping skills, independence, and quality of life. This role works closely with residents, families, clinical staff, and community providers to support individualized behavioral health goals and recovery-focused care.

Key Responsibilities:

• As a psychiatric rehabilitation services coordinator (PRSC), you will review and assist the resident in understanding the treatment plan.
• You will prepare and assist the residents with active participation in the treatment plan review.
• You will provide and/or coordinate the delivery of the psychiatric rehabilitation services programs.
• You will re-evaluate the residents on a quarterly basis.

• Demonstrated sensitivity, ability and skills in working with and understanding the needs of the residents, family members, staff members and personnel from community agencies.
