• Demonstrated ability to work collaboratively, maintain professional boundaries, and support recovery-oriented care.

• Support clients in managing their healthcare needs, including scheduling, follow-ups, wellness planning, and health literacy.
• Conduct regular home visits to evaluate and reinforce safety, cleanliness, and daily living skills.
• Promote community inclusion by connecting clients with resources, educational opportunities, and natural supports.

Position Summary

The Psychiatric Rehabilitation Specialist plays a key role in supporting clients on their recovery journey. Using the principles of Psychiatric Rehabilitation—including empowerment, self-determination, healing-centered, community integration, skill development, and a strengths-based approach, the Specialist helps clients identify meaningful life goals and develop the skills, supports, and confidence needed to achieve them. This position includes shared responsibility for the caseload within a collaborative team environment.
