Job summary

Allure of Moline in East Moline, IL is seeking a Psychiatric Rehabilitation Services Coordinator (PRSC) to oversee the Social Services Department and assure the residents’ emotional and social needs are met in accordance with applicable standards and policies.

You will collaborate with an interdisciplinary team, develop care plans, document progress, and support discharge planning while providing education to residents, families, and staff.

Qualifications

• Bachelor’s degree in a human services field.
• Minimum of one year of supervised experience in mental health or human services preferred.

Responsibilities

• Review facility policies to ensure compliance with regulations.
• Participate in policy development for resident care and quality of life.
• Document progress in meeting residents' psychosocial needs.
• Coordinate discharge planning and referrals for home care.
• Provide groups for residents and families.
• Develop written interdisciplinary plan of care for residents.
• Deliver therapeutic interventions to support residents’ transition.
