The role emphasizes advising on resident psychosocial needs, coordinating care plans, supervising social work staff, and ensuring compliant documentation, discharge planning, and family education.

Qualifications

• Bachelor's degree in a human services field required or equivalent.
• Minimum one year of supervised experience in mental health or human services preferred.

Responsibilities

• Review policies and procedures with the interdisciplinary team to ensure compliance with regulations.
• Develop, implement and supervise care plans addressing residents' psychosocial needs.
• Coordinate discharge planning and referrals for home care services.
