Job summary

Jewish Family & Children's Service of Arizona seeks a Psychiatric Mental Health Nurse Practitioner to provide adult psychiatric evaluations, medication management, and annual updates. You will collaborate with primary care providers and the adult clinical team to coordinate comprehensive care, document in the EMR, and obtain informed consent when prescribing medications.

Qualifications

  • PMHNP certification by AZ Board is required.
  • Active AZ license and DEA registration.
  • NPI designation required.
  • Minimum 2 years in behavioral health.
  • FingerPrint clearance card without restrictions.
  • Valid AZ driver's license and reliable transportation.
  • Ability to use EMR and telehealth for visits.

Job description

Critical Tasks
  • Provides psychiatric evaluations, medication management and annual psychiatric updates to adults (see employee training and licensure)
  • Provides consultation and clinical leadership for all members of the adult clinical team
  • Works in collaboration with the client Primary Care Provider on shared clients in order to facilitate a whole health approach
  • Maintains documentation in client medical record per licensure and JFCS requirements
  • Order labs per standard of practice as medically necessary when prescribing medication
  • Obtain medical informed consent from clients or (guardians where stipulated) when prescribing medication
  • Maintains professional communication for purposes of coordination of client care with other agencies, PCPs, hospitals, EDs and/or urgent cares
  • Practices in a manner that is in accordance with current ethical standards and promotes a positive public image for JFCS and the profession
  • Will practice in accordance with all applicable state and federal laws and the professional standards
Key Performance Indicators
  • Demonstrated ability to work in a multidisciplinary SMI team environment to coordinate care on behalf of clients
  • Provide clinical guidance to multidisciplinary SMI team during daily morning meeting
  • Demonstrates excellent written and verbal communication skills with clients
  • Has the ability to tailor their communication to fit the needs of diverse populations
  • Demonstrated ability to identify barriers and challenges to care and ability to resolve these barriers/challenges in a positive, culturally appropriate solution focused manner
  • Provides a positive experience for clients when providing services
  • Works closely with their chief medical officer when treating complex clients including court ordered and high risk individuals.
Metrics
  • Maintains productivity standards as established by JFCS
  • Documents in the medical record all services provided within 24-48 hours
  • Completes documentation of informed consent when prescribing psychiatric medication 95% of the time
  • Completes judicial reviews, renewals and amendments when required for court ordered treatment
  • Monitors visit adherence for assigned individuals on court ordered treatment
  • Responds timely in terms of SMI crisis client, especially where court ordered treatment exists
  • Orders labs per standard of practice 95% of the time
  • Sees clients within half hour of their arrival for scheduled appointment time 90% of the time
Qualifications
  • PMHNP by Arizona Nursing Board
  • License must be current and in good standing to practice in Arizona
  • Required full prescription authority and maintain current DEA licensure
  • Required NPI designation
  • Minimum of 2 years’ experience operating in a behavioral health setting or demonstrated equivalent experience
  • Level 1 fingerprint card without restrictions
  • Reliable transportation, meet agency requirements for Motor Vehicle Record Report
  • Valid Arizona Driver’s License/auto insurance
  • Ability to utilize an electronic medical record and utilize telehealth platform to conduct virtual visits if clinically indicated
  • Must be able to perform the essential functions of the job including prolonged sitting and in certain positions driving, lifting 20lbs, walking, standing and executing dexterity with a keyboard or smart pad.
