Job summary

UltaCare Medical is seeking a compassionate Psychiatric Mental Health Nurse Practitioner to provide autonomous, high-quality psychiatric care in post-acute settings. The role emphasizes collaboration with an interdisciplinary team and a flexible, rewarding career with competitive pay.

You will manage anxiety, depression, and other conditions, maintain thorough documentation, and educate facility staff while adhering to CMS guidelines. Experience with geriatric populations is preferred.

Qualifications

• Experience caring for geriatric populations or patients with dementia preferred.
• Experience working in post-acute or healthcare settings preferred.
• Knowledge of standard documentation practices.
• Unrestricted PMHNP license

Responsibilities

• Optimize quality of life for residents in skilled nursing, assisted living and long-term care settings with an interdisciplinary team.
• Assess, diagnose, treat and medically manage PMH conditions including anxiety and depression.
• Maintain timely, accurate documentation.
• Provide education to facility staff regarding psychiatric care and patient needs.
• Ensure CMS guidelines and facility policies are followed.

• Optimize quality of life for residents in skilled nursing, assisted living and/or long-term care settings working alongside an interdisciplinary team that includes medical professionals, psychotherapists and facility staff.
• Assessment, diagnosis, treatment and medical management of conditions including anxiety, depression, adjustment disorder, and pre-existing mental illnesses.
• Maintain timely, accurate documentation.
• Provide education to facility staff regarding psychiatric care and patient needs.
• Ensure compliance with CMS guidelines and facility policies
