Job summary

UltaCare Medical seeks a Psychiatric Mental Health Nurse Practitioner to join our post-acute care team. The role focuses on autonomous practice within skilled nursing and assisted living facilities, delivering comprehensive psychiatric care and supporting residents with complex mental health needs.

You will collaborate with an interdisciplinary team to optimize quality of life, perform assessments, provide education to staff, and ensure CMS guideline compliance.

Qualifications

• Unrestricted PMHNP license.
• Experience with geriatric population and dementia preferred.
• Experience working in a post-acute or healthcare setting preferred.

Responsibilities

• Optimize quality of life for residents in skilled nursing, assisted living and long-term care settings with an interdisciplinary team.
• Assessment, diagnosis, treatment and medical management of conditions including anxiety, depression, adjustment disorder, and pre-existing mental illnesses.
• Maintain timely, accurate documentation.
• Provide education to facility staff regarding psychiatric care and patient needs.
• Ensure compliance with CMS guidelines and facility policies.
