Postgraduate-Center-For-Mental-Heal is seeking a Psychiatric Nurse Practitioner to provide direct community-based mental health services. You will participate in team and morning meetings, assess medication needs, educate clients and families on medications, and document all actions in the electronic health record.

• Master's degree in advanced nursing from an accredited program.
• Current NYS license as a Psychiatric Nurse Practitioner and NYS RN license.
• Enrolled in NYS Medicaid; PECOS enrollment preferred.
• Minimum of three years working with serious mental health conditions, co-occurring substance use disorders, and homelessness.

Responsabilidades

• Participate in morning and staff meetings and coordinate care with team.
• Evaluate need for psychotropic medications, educate clients and relatives, adjust doses, and document in electronic chart.
• Complete psychiatric evaluations and updates; conduct visits in the community per OMH licensing standards.
• Coordinate medication access with RNs/LPNs and administer injectable medications when needed.
• Order routine blood work and/or toxicology testing as needed.

JOB SCOPE:This is a direct service position requiring specific knowledge of mental health, and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires the desire to work as part of a team.ESSENTIAL FUNCTIONS:Participate in morning meetings and staff meetings.Evaluate need for psychotropic medication; prescribe medications as needed, educates clients and relatives about these medications and their effects, reviews and adjusts dose level as needed, and documents all medications in consumer’s electronic chart.Complete pyschiatric evaluations and updatesConduct visits in the community in accordance with NYS Office of Mental Health (OMH) licensing standards.Coordinate consumer access to medications with the RN and LPN nurses including administering injectable medications when needed.Order routine blood work and/or toxicology testing as needed.EDUCATION AND EXPERIENCE:Master's degree in advanced nursing from an accredited Nurse Practitioner Program; Current license in NYS as a Psychiatric Nurse Practitioner; NYS license as a Registered Nurse.Enrolled in NYS Medicaid required. Enrolled in PECOS, preferred.Minimum of three (3) years working with people with serious mental health conditions, co-occurring substance use disorders, and homelessness.
