Job summary

Postgraduate Center for Mental Health seeks a Licensed Registered Nurse in New York to provide direct service in the community. The role emphasizes crisis intervention, medication coordination, and teamwork within an ACT program.

Travel to community sites is required, with participation in team meetings and ongoing patient care. The ideal candidate has NY RN licensure and experience with mentally ill populations, including MICA, and is able to support nursing assessments and daily progress notes

Qualifications

• Licensed Registered Nurse in New York with an Associate's, Bachelor's, or Master's degree.
• Experience serving the mentally ill or MICA population is preferred.

Responsibilities

• Participates in daily morning meetings.
• Assists RN with administering IM injections and preparing medications for weekly delivery.
• Maintains monthly IM calendar.
• Coordinates with prescribers and pharmacy to ensure meds and refills.
• Transcribes orders and updates MARs.
• Logs medication deliveries and completes tox request forms.
• Completes nursing assessments and daily progress notes.
• Conducts weekly Healthy Living group with consumers.
• Performs other duties as directed by Team Leader.

JOB SCOPE

This is a direct service position requiring specific knowledge of mental health, and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.

ESSENTIAL FUNCTIONS

• Participates in daily morning meeting.


• Assists RN with administering IM's and preparing oral medications for weekly delivery by the clinical team.


• Assists RN with maintaining monthly IM calendar.


• Coordinates with Team prescribers and pharmacy to ensure consumers receive medications and refills.


• Transcribe orders and updates Medication Administration Records.


• Assists with logging in medication deliveries and completing toxicology request forms.


• Assists with completion of annual nursing assessments and daily progress notes.


• Conducts weekly Healthy Living group with consumers.


• Performs other duties, consistent with the goals and objectives of the program, as directed by the Team Leader.

EDUCATION AND EXPERIENCE

• Licensed Registered Nurse in New York in conjunction with either an Associate's, Bachelor's or Master's degree.


• Experience serving the mentally ill/mentally ill chemically addicted (MICA) population.

WORK ENVIRONMENT

This position operates 80 percent of the time in the community, rotating in an office setting as needed and for team meetings.
