Albuquerque Health Care for the Homeless is seeking a Psychiatric Nurse Practitioner to deliver psychiatric care for clients experiencing homelessness, in our clinic and mobile outreach teams in Bernalillo County. A strong background in trauma-informed care and harm reduction is preferred to support our holistic, client-centered approach.

Qualifikationen

• MSN or DNP from accredited program; APRN-CNP or PMHNP-BC licensed.
• DEA and CSR license; experience documenting services in an EHR.
• Experience providing psychiatric care in a medical/clinical setting; thrive in integrated, cross-disciplinary teams.

Aufgaben

• Work with other disciplines toward integrative approaches to patient psychiatric care.
• Provide psychiatric care to those experiencing homelessness at clinic sites and outreach locations.
• Serve as cross-department psychiatric resource for AHCH staff.
• Collaborate with staff in treatment planning.
• Participate in planning, case reviews and quality assurance activities.
• Participate in peer review processes.
• Maintain standards of psychiatric care per protocols.
• Prescribe and monitor psychiatric medications as needed for AHCH patients.
• Work closely with primary care and behavioral health providers on psychiatric clients.
• Ensure timely documentation of services in the EHR.
