Albuquerque Health Care for the Homeless is seeking a dedicated Nurse Practitioner to provide holistic care to clients experiencing homelessness. The ideal candidate should possess a Master's degree in Nursing and relevant licenses, along with expertise in chronic disease management and trauma informed care.

Since 1985, Albuquerque Health Care for the Homeless has dedicated its mission exclusively to providing services to people experiencing homelessness. AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. Our vision is to live in a world that is just and without homelessness. We believe that homelessness is an issue that can be solved through access to quality health care, adequate and affordable housing, and a living wage.
We are currently looking for a 0.8 FTE Nurse Practitioner who is passionate about working with our clients. Our ideal candidate will be adept in chronic disease management and substance use disorder treatment.
Our treatment model is integrated with other departments and agencies and the candidate would need to be a strong team player. We believe that there are many pieces to a person's well-being and we need a candidate with this holistic approach. The candidate would need to be comfortable in the clinic as well as treating in part of our mobile teams; we have a treat first model that incorporates on site treatment in outreach settings. Our ideal candidate would have working knowledge of harm reduction philosophies and would be able to embody these ideas with clients.
Compassion for and an understanding of the wide variety of issues for those experiencing homelessness is a must. Trauma is an underlying issue with most of our clients and we need a candidate that has a working knowledge of trauma informed care. Our clinic operates in a shared client model so the ideal candidate would be comfortable sharing clients with multiple practitioners.
