Job summary

The Buffalo VA Medical Center is seeking a Licensed Practical Nurse to join the Nursing, Mental Health team. The LPN provides holistic care to patients in acute to chronic stages of illness as part of an interdisciplinary team.

You will participate in planning, implementing, and evaluating nursing care, document data, and assist with procedures. Administer medications, ensure safety, and maintain a clean environment.

Qualifications

• Current LPN license and registration.
• Experience in acute, sub-acute, or chronic care.
• Ability to communicate effectively with team and patients.
• CPR/BLS certification required.

Responsibilities

• Plan, implement, and evaluate nursing care with the team.
• Maintain patient/family/staff relationships via communication.
• Document data and participate in shift reports and care planning.
• Collaborate with RN to develop individualized care plans.
• Administer medications and monitor effects.
• Assist in diagnostic procedures and patient safety.
• Maintain a safe, clean environment and report hazards.
• Document in medical records and provide patient teaching.
• Prepare for procedures and respond to emergencies with CPR.
