Job summary

Meritus Health is seeking a Clinical Pharmacist Specialist to provide comprehensive pharmacotherapy services for psychiatric/behavioral health patients. The role includes medication optimization, therapeutic monitoring, education, and formulary stewardship within an integrated team.

You will develop and maintain specialized clinical practice areas, mentor pharmacy staff, and support patient safety and quality initiatives in a dynamic health system setting.

Qualifications

• PharmD degree or equivalent combination of education and experience required.
• Maryland pharmacist license required, or eligibility to obtain Maryland licensure.
• Board Certification Specialist (BCS) certification is required within three years if not already obtained.
• KATA or other Lean certification must be obtained within one year.
• Newly graduated pharmacists pending Maryland licensure must obtain Maryland licensure within four months of graduation or hire date.

Responsibilities

• Prepare and dispense medication orders according to policies and protocols.
• Oversee pharmacy personnel involved in preparation and dispensing.
• Ensure compliance with USP requirements for sterile compounding.
• Monitor medication therapy for interactions, contraindications, and dosing.
• Promote safe and cost-effective medication therapy per protocols.
• Participate in antimicrobial stewardship and medication management programs.
• Collect medication histories and reconcile medications.
• Respond to Code Blue and Rapid Response Team events.
• Participate in formulary management and adherence to policies.
• Provide accurate drug information to medical staff.
• Educate staff, patients, and families on medication topics.
• Document clinical activities and interventions.
• Identify and report adverse drug reactions and medication errors.
