Job summary

Wexford Health Sources is seeking a Psychiatric Licensed Practical Nurse (LPN) to support mental health patients at Central NM CF in Los Lunas, NM. The role involves under RN guidance delivering nursing and behavioral health care, medication administration, and patient education.

You will perform assessments, monitor vital signs, document encounters, and collaborate with nurses, providers, correctional staff, and the treatment team to promote stability and well-being.

Qualifications

• Graduate of an accredited nursing program.
• Current, unrestricted LPN license in the state of practice.
• Current CPR certification.

Responsibilities

• Assist with physical, psychological, and social assessments and individualized treatment planning.
• Administer medications and implement treatment plans per provider orders.
• Monitor patients, take vital signs, provide basic nursing care, and respond to needs.
• Document nursing encounters and patient responses in the medical record.
• Collaborate with RNs, providers, correctional staff, and the treatment team.
• Support patient education, activities of daily living, infection control, and a therapeutic environment.

PAY RATE: $3000 Sign-on Bonus

As a Psychiatric LPN, you'll work alongside registered nurses, providers, and behavioral health professionals to help patients achieve greater stability, safety, and well-being.

Position Summary

Under the clinical guidance of a Registered Nurse, you'll assist in delivering nursing and behavioral health care through assessment, planning, implementation, evaluation, medication administration, and patient education. Your responsibilities will include:

• Assisting with physical, psychological, and social assessments and individualized treatment planning
• Administering medications and implementing medical and mental health treatment plans in accordance with provider orders
• Monitoring patients, taking vital signs, providing basic nursing care, and responding to physical and emotional needs
• Documenting nursing encounters, patient responses, complications, and other relevant clinical information
• Collaborating with registered nurses, providers, correctional staff, and other members of the treatment team
• Supporting patient education, activities of daily living, infection control, and a safe, therapeutic environment

You'll go beyond providing routine nursing care. You'll offer consistency, compassion, and clinical support to patients navigating complex mental health challenges.
