Job summary

Promise Resource Network is seeking a Community Inclusion Specialist in Charlotte, NC. The role focuses on transitioning TCL-eligible members from Adult Care Homes to Permanent Supportive Housing and connecting them to services to enhance life quality and community tenure.

The CIS will work with IPS-SE, Benefits Counseling, and other supports, requiring commitment to trauma-informed, recovery-focused care and peer-support principles within a nonprofit setting.

Qualifications

• Current North Carolina Certified Peer Support Specialist.
• Lived experience with substance use disorder and/or mental health challenges, meeting eligibility for NC PSS certification.
• Belief in recovery potential and commitment to peer support principles.
• Ability to receive guidance, training, and mentoring to grow skills.
• Demonstrates leadership by modeling PRN’s mission and values.
• Takes personal responsibility, is dependable, reliable, and a team player.
• Honesty and integrity; ethical and approachable.
• Ability to interact effectively with the public, providers, and stakeholders.
• Commitment to personal recovery and wellness with lived crisis-based experience.
• Ability to manage time and complete tasks.
• Knowledge of computer software: word, spreadsheets, databases, presentations.
• Honesty and integrity reiterated.
• Excellent communication skills in oral and written forms; active listening and conflict resolution.
• Must have a valid NC Driver’s license.

Responsibilities

• Follow referrals from the Program Manager for TCL members in Community Inclusion programs.
• Educate members about housing, services, supports, and opportunities to promote community inclusion.
• Complete IDM Tools and Community Participation Assessments; coordinate opportunities based on needs and preferences.
• Use the Mattering framework to improve quality of life for members.
• Communicate with the Program Manager Alliance to coordinate housing transitions and outcomes.
• Complete and submit required documentation for reporting and billing as needed.
• Adhere to System of Care values in person-centered service delivery to achieve outcomes.
• Attend weekly program meetings as facilitated by the Program Manager.
• Schedule planning meetings with the member, guardian, and facility as needed.
• Conduct face-to-face or virtual visits with members and guardians.
• Help members connect with peers and community resources.
