Qualifications

  • Bachelor's degree or equivalent in Business, Public Health, Healthcare, or related field.
  • 3+ years in healthcare industry, preferably in provider practice management, quality, medical assisting, coding, or revenue cycle.
  • At least 1 year of experience with Medicaid/Medicare value-based care and population health management.
  • Experience with HEDIS and STARs quality measures.
  • Knowledge of medical coding, billing, and revenue cycle processes.
  • Experience with EHR/EMR systems.
  • Demonstrated ability to drive practice transformation and performance improvement.
  • Healthcare quality certification such as CPHQ is preferred.
  • Medical Assistant credentials or experience are preferred.
  • Experience with EMR template development is preferred.

Responsibilities

  • Support provider practices in implementing value-based care programs and performance improvement strategies.
  • Improve access to care, preventive care, chronic condition management, and overall quality outcomes.
  • Support efforts to reduce avoidable hospital and emergency department admissions and readmissions.
  • Improve appropriate medical coding, documentation, and billing practices.
  • Align provider practices with quality and performance goals.
  • Establish and monitor weekly, monthly, and annual operational performance goals.
  • Evaluate practice behavior changes and quality performance against established targets.
  • Analyze population health data and present performance results to practice and organizational leadership.
  • Identify barriers to performance and recommend practical improvement strategies.
  • Drive sustainable operational and behavioral change through practice transformation.
  • Monitor, document, and report practice progress and recommend next actions.
  • Support effective use of population health management tools and digital data systems.
  • Facilitate implementation of digital data retrieval and integration processes.
  • Train and support provider practices on performance improvement and value-based care strategies.
  • Build strong relationships with physicians, practice leadership, and clinical staff.
  • Collaborate with internal teams and maintain feedback loops regarding provider performance and improvement activities.
