Job summary

P3 Health Partners is seeking a Provider Operations Coordinator in Arizona to manage operational initiatives and oversee special projects focused on HEDIS quality gap closures, risk adjustment activities, and provider engagement. You will coordinate across departments to support clinics and educate patients about preventive screenings.

The role requires strong communication, organizational, and analytical skills with healthcare experience and EHR proficiency preferred.

Qualifications

• High school diploma/GED required; associate degree preferred.
• 2+ years in healthcare-related fields; clinical health care or health plan experience preferred.
• Experience with Microsoft Word, Excel, PowerPoint, Outlook and office equipment.
• Electronic Health Records experience required.
• Medical Assistant Certification or related certification preferred.
• Experience with data mining preferred.
• Medical terminology knowledge required; CPT II codes preferred.
• CPR Certification preferred.

Responsibilities

• Build and maintain relationships with Affiliate groups; serve as liaison between groups and departments.
• Advise and support affiliate practices with quality gaps and documentation via outreach, data audits, and claims surveillance.
• Prioritize patient outreach and coordinate scheduling for new and established patients.
• Coordinate care across hospital discharge, care management, referrals, and quality initiatives.
• Assist in CSR/IDT/JOC meeting prep and participate as requested.
• Conduct data entry and maintain documentation for quality initiatives.
• Educate patients and affiliates on preventative screenings.
• Potential in-home HEDIS gap closure for certain measures.
• Other duties as assigned.
